LLCs should include LLC, L.L.C., or similar designations in the name. Corporations should include Corp, Inc., Incorporated, or similar designations in the name.

Your business address should be the main, physical location you will have your business set up when it is formed. A house or an apartment is fine (make sure you are allowed to run a business from your home in your city), but this should not be a PO box.

If you get your mail at a different address than your business address, please include that as well, but as a separate location.

Owners of corporations are called "shareholders." Shareholders own "shares" of the corporation.
